Passports Request

Documents to be submitted

1- Updated Identity card or birth certificate duly recognized by the Notary and authenticated by the Ministry of Foreign Affairs.

2- Three (3) updated passport photographs in color with white background.

3- For students, submit the statement from the school; for workers, submit the statement from the employer.

4- Fill in, with black ink and block letters, the form and index card provided by the Embassy.

5- If you are under 18, submit the Liability of parents or one of the parent including their personal documents, Identity Card or Passport.

6- Proof of payment of consular fees.

Payment: USD 100.00 by bank transfer to the Embassy's Account.
